Insurance information is required on the patient requisition form. Please also sign the ‘Authorization to Release Information and Pay Benefits’ section. Front and back copies of insurance cards are also needed. Patients may be liable for additional fees such as deductibles, contract exclusions, non-covered services, co-payments, and co-insurance.

Retrogen can directly bill a referring institution. Please contact us to obtain information about setting up an institutional account.

We accept checks, money orders, credit cards (AMEX, Discover, Mastercard, or Visa), and wire transfers. Please make checks and money orders payable to Retrogen Diagnostics. For credit card payments, include card type, cardholder name and signature, account number, expiration date, and CVC security code.
